Unlimited Doula Visits
FCC offers pregnant members unlimited doula visits.
MOMitor App
This is an app for high-risk pregnant members. The app is a tool that supports mothers during pregnancy and after delivery to look for early signs of common health problems that can occur.
Home Visiting
FCC has partnered with mobile providers to offer services in member's home.
Enhanced Care Coordination
FCC offers enhanced care coordination to pregnant members as needed.
Home Delivered Meals
FCC offers high risk pregnant members with home delivered meals during pregnancy to offer unlimited nutritional support. This includes one 10-pack of meals per week for the duration of their pregnancy.
Lactation Support
FCC offers members 24/7 access to virtual lactation and pregnancy support up to six months after delivery.
Breast Pump Coverage
FCC members receive one breast pump per lifetime.
FCC is here to help you plan for your family, support you during your pregnancy, and provide care during your postpartum recovery. We are here for you!
In addition to the rewards listed below, members receive a $50 incentive for notifying FCC within 30 days of positive pregnancy test. Notifying us is easy, just fill out the notification of pregnancy form and email to fccobteam@fcchealthplan.com.
We also want to reward you for receiving routine care during and after your pregnancy. If you complete any or all of the activities below, you can receive rewards in the dollar amount shown! If you have any questions, call our member services team at 1.833.FCC.PLAN (1.833.322.7526), or 711 for TTY Monday through Friday from 8 a.m. to 8 p.m. Eastern.

Notice of Pregnancy | Tell us you are pregnant within 30 days of confirmation of pregnancy by your doctor | $50 |
First Prenatal Visit | Attend your first prenatal visit with your doctor within your 1st trimester | $50 |
Prenatal Visits | Attend additional prenatal visits with your doctor throughout your pregnancy | $25 per visit, up to a maximum of $100 |
Postpartum visit | Attend a postpartum visit with your doctor within 7 - 84 days of delivery | $25 |
Well-child visit | Bring your baby for a Pediatrician visit within 30 days of birth | $25 |
