Do you qualify for SMMC Long-Term Care Program?
Qualify for SMMC Long-Term Care program (Nursing Home Level of Care)
The SMMC Long-term Care (LTC) program provides nursing facility services and home and community-based care to elders and adults (ages 18 years and older) with disabilities. Home and community-based services help people stay in their homes, with services like help with bathing, dressing, and eating; help with chores; help with shopping; or supervision.
NOTE: If you qualify for the Long-Term Care program, the Agency for Health Care Administration (AHCA) will send you a letter. The letter will ask you to choose a plan. Choose Florida Community Care!

To choose a plan, go to the state enrollment broker website or call a choice counselor.
