Medicaid Managed Long Term Care for Medicaid enrollees 18 years and older who meet Nursing Facility level of care. Florida Community Care is working with Medicaid and Long Term Care members to improve their health status and quality of life. Florida Community Care cares for members by providing quality services to meet the care needs of members.

Florida Community Care’s recognizes the vital relationship between a member and a trusted network of providers. This relationship is critical to ensuring members make the most effective use of managed care. Florida Community Care is dedicated to selecting providers most suited to care for its members based on proximity, languages spoken, cultural similarities, dedication to quality, and areas of specialty.

If you are a contracted Florida Community Care provider, you can register now. If you are a non-contracted provider, you will be able to register after you submit your first claim.

Once you have created an account, you can use the Florida Community Care provider portal to:

  • Verify member eligibility
  • Manage claims
  • Manage authorizations
  • View patient list
  • Login/Register
