Disease Management Program
Programs If you have a chronic condition, we work with you and your caregivers, to provide you with disease management services. We want to help you lead a healthier lifestyle, so we reach out over the phone, through educational tools and with support. This way, you can control your condition better, understand it more and have fewer complications.
How does our Disease Management Programs help you?
Florida Community Care’s Disease Management Programs have a team of clinical staff with experience helping members who live with a chronic illness.
Our programs for chronic care include educating you about your conditions. We also teach you self-management tools, and help you reach your goals.
What Disease Management programs are offered?
Asthma/COPD
Asthma is a lung disease that causes the inside of the airways to get swollen and the muscles to get tight. Asthma may cause you to have trouble breathing. You may also get shortness of breath and you may feel weaker.
COPD (Chronic Obstructive Pulmonary Disease) can also be known as chronic bronchitis and/or emphysema. Bronchitis is swelling of the airways in the lungs. Emphysema is damage to the air sacs in the lungs.
Cancer
Cancer is a disease in which cells grow and divide with little or no control. There are many types of cancer, they normally are called by the organ or cell where the cancer begins.
Dementia/Alzheimer's
Dementia is loss of mental function that will affect the ability to carry our everyday tasks. Dementia affects memory, ability to do everyday tasks, and ability to communicate.
Alzheimer’s disease is a disease that gets worse with time. This disease affects memory, language, and thought. The exact cause is still not known.
Diabetes
Diabetes is a chronic disease in which your body does not make enough insulin (Type I) or the insulin in your body does not work the way it is supposed to work (Type II). This will cause high levels of sugar in your blood. Diabetes worsens with age and with other health conditions that may be present
Hypertension
Hypertension or High Blood Pressure is when the force of your blood in your arteries is always high. This can cause damage to the walls of your arteries. This high force, if always constant, will cause the heart and lungs to work harder than normal and cause damage to the heart and lungs. It is important to lower the force so that the work on the heart and lungs is also decreased. This can be done by medicine, diet, exercise, decreasing stress, and/or losing weight.

These chronic diseases can have an impact on your quality of life. There is also added stress on your family members and caregivers.
You may start finding it difficult to do everyday activities. Activities that were once easy to do. It is important to manage this condition. With the support of the FCC team, you can learn how to report and control your symptoms. By managing some of the causes, you can help prevent the diseases from getting worse.
